For Right to Choose patients
Generally, the patient will have an idea of which provider they have in mind. Each provider has their own requirements, however generally they will require reason for the referral as well as an ASRS.
This information is included in the patient’s Think ADHD PDF report, so the PDF can be supplied as an attachment.
