How to Refer

For Right To Choose - Your GP must refer via email or ERS.
There is a referral form which is optional for GPs.

When it comes to titration, we will continue to see you for as many appointments as you need. There is no fixed limit, and we won’t discharge you after just a few sessions. Our goal is to work with you until your medication and symptoms feel well‑managed and stable.

Your ADHD Assessment Pathway

To get an ADHD assessment with HealthHarmonie Minds, you will need to get a referral from your GP.
Through the right to choose scheme, we make the process straightforward and support you every step of the way.

Pre-assessment

This stage gathers initial information to determine whether an assessment is appropriate.

Referral

Initial referral for ADHD assessment (by healthcare professional).

Pre-assessment questionnaires

Patient completes pre-assessment questionnaires.

MDT review

Multidisciplinary Team (MDT) reviews case notes and questionnaires to confirm next steps.

Assessment

This stage evaluates the patient’s history and symptoms to reach a diagnostic outcome.

Assessment part A

First assessment appointment using the DIVA-5 tool to evaluate symptoms.

Assessment part B

Diagnostic assessment with an appropriately qualified ADHD clinical assessor.

Post-diagnosis

There may be a number of pathways – these may be with or without medication.

Path A: Pharmacological support

Titration appointment

Start of medication titration process.

Titration 1st follow-up

First follow-up for titration.

Titration 2nd follow-up

Second follow-up for titration.

Path B: Non-pharmacological support

Psychoeducation

Group sessions to help individuals understand ADHD and develop self-management strategies.
