Standard Operating Procedure for ADHD referrals

Standard procedure for patients requesting an assessment for ADHD:

  1. Patient requests ADHD assessment through traiage system, reception call or during a GP consultation.

  2. Send Accurx text or similar linking the Think ADHD screening tool.

  3. Patient to complete screening tool and receive PDF with completed information to their nominated email address.

  4. Patient returns PDF to surgery through text message return link/email/printed (delete as appropriate for your practice).

  5. PDF is reviewed and included with a covering letter to the ADHD assessment service.

Right to Choose

For Right to Choose assessments, the PDF can be sent along with the specific paperwork requested by the service in question.

This will differ according to the service, however they will generally require:

  1. Completed ASRS (this included in the patient’s Think ADHD PDF report)

  2. A completed referral document. The patient’s Think ADHD PDF report will supply information pertinent to completing this document.

  3. In some cases, a Patient Waiting List Risk Assessment.

Private Assessment

For individuals requesting a private assessment, the patient’s Think ADHD PDF report can be sent along with a simple covering letter requesting private ADHD assessment.
